Provider Initiated Testing

In addition to our direct access testing we offer provider-initiated bloodwork services for adult patients with valid provider orders.

To submit a lab request, please email a completed and signed laboratory requisition form including the following information:

• Provider’s name, contact information, license number, and email

• Patient’s full legal name, date of birth, contact information, and email

Once received, our team will contact the patient within one business day to coordinate scheduling.
