Provider-Initiated Testing (PIT)
Blood tests fall into two categories: Direct Access Testing (DAT) and Provider-Initiated Testing (PIT).
Direct Access Testing (DAT)
Many blood tests we offer are Direct Access Tests (DAT). This means anyone can order these tests themselves without needing a Provider’s order.
With DAT:
You can purchase the test directly through our Online Ordering page and schedule your blood draw appointment.
Results are released directly to you once the lab work is complete.
Provider-Initiated Testing (PIT)
By law, some tests must be ordered by a licensed medical provider only. These are called Provider-Initiated Tests (PIT).
For PIT testing, a licensed provider must submit a requisition form to the draw center before the blood draw can be performed. The requisition form must include:
The specific tests being ordered
The medical reason for the test
The provider’s license number
Patient information
Once we receive a completed Requisition Form, we can proceed with the blood draw. Providers are welcome to submit their own form, or they can fill out ours.

Release of PIT Results
When PIT test results are completed, our policy is to release the results simultaneously to both:
The ordering provider, and
The patient
If different release arrangement are preferred, those must be noted in writing.
Can you do a PIT blood draw without a provider’s order?
No. A completed Requisition Form must be received before a PIT blood draw can be performed. We cannot draw blood first and wait for a provider to submit the order afterward.
Pricing Transparency
For transparency, PIT tests pricing is listed on our website test directory. However:
PIT tests cannot be ordered through the Online Ordering page.
PIT tests can only be ordered by our team after reviewing a completed requisition form from a licensed provider.
